| We are a not-for-profit organization in the USA dedicated to patient safety. Health care has been amazingly successful at turning public debate into lawsuits. The founder of this site is being sued. That has so spooked this board that it is not functioning. The founder is continuing the work on his own outside of this organization. Below is what we were working on previously and plan to resume working on if the air ever clears.
Current WorkWe are looking at information systems. How and when is information recorded? What filters does it go through before it reaches someone like a CEO who might be setting policy for an entire hospital. What happens to complaints filed by patients? What happens to incident reports filed by doctors? What do administrators see when they try to get an overview of their organization?
Immediate ProjectsProject 1) We are asking hospitals about the systems they use to track patient complaints. Some simply forward complaints to the department in which the problem occurred, providing no opportunity for oversight by anyone outside that department. Project 2) Nurses are supposed to be the patient's advocate. We are circulating a series of surveys to learn more about how and when nurses give voice to patient safety issues. Preliminary results indicate that no nurse ever reports an incident to anyone outside the place where he/she works. Since what is reported inside cannot be discovered by anyone outside, even through legal action, even in a state with mandatory reporting laws, it means that mandatory reporting initiatives may not be a solution, as at least one board member of the AMA predicted.
